One zirconia crown was placed by Dr. Shimizu on tooth #11 (upper left canine).

Emax veneers were placed by Dr. Shimizu on teeth #6–9 and #11 (upper right canine to upper left central incisor and upper left canine). An Emax crown was placed on tooth #10 (upper left lateral incisor).

A dental implant was placed by Dr. Shimizu on tooth #7 (upper right lateral incisor). Emax veneers were placed on teeth #5, 6, and #8–12 (upper right first premolar, upper right canine, and upper left central incisor to upper left first premolar).

A zirconia bridge was placed by Dr. Shimizu spanning teeth #7, 8, and 9 (upper right lateral incisor to upper left central incisor).

Composite bonding was performed by Dr. Shimizu on tooth #8 (upper right central incisor), and a dental implant was placed on tooth #9 (upper left central incisor).
